Care Guide Partners PACE (Program of All-Inclusive Care for the Elderly) redefines elderly care by offering a comprehensive program that supports the elderly in maintaining their independence and quality of life in their own homes. This innovative approach combines healthcare, rehabilitation, and social services, tailored to each individual's needs.

Service Area: Bullitt, Grayson, Hardin, Hart, LaRue and Nelson counties
Participants: Designed for 200 – 250 participants
Fundraising Goal: $3 million raised

PACE is dedicated to serving one of our community's most vulnerable populations—older adults. By providing a network of direct services and community providers, PACE ensures that participants receive the care they need to stay in their homes, promoting independence while offering peace of mind to their families.
